What You’ll DoProvide skilled nursing care in home health settings 🩺
Support patients to remain safely in their homes while promoting independence and wellness 🌟
Collaborate with an interdisciplinary team to deliver comprehensive care
Travel within your assigned territory, with federally reimbursed mileage 🚗💨
Completion of an accredited LPN program
Current LPN licensure or compact licensure recognized in Vermont
2+ years of home health, public health, or hospice experience preferred
Previous home care experience is a plus, but not required – mentoring provided 🌱
Ability to walk, stand, bend, lift, and move intermittently during shifts
Lift, push, or pull up to 50 lbs safely
Navigate stairs, maintain balance, kneel, crouch, twist, and reach
Manual dexterity and visual/auditory acuity for patient care and equipment use 👀🎧
